Welcome to the 2017-2018 Dodge County Youth Hockey
& Four Seasons Mite Hockey Registration Site!  

 

All players must register online.  Below is information about registration.  Log-in, scroll to the bottom and click on the button to register.

All forms that need to be submitted to the registrar along with this online registration can be downloaded within the document below.  There are also individual forms available below which match this comprehensive packet.  

 

All forms and checks can be put in the DCYH drop box at the Kasson rink or mailed to:

turned in on Thursday, September 14th 2017 in person:

  • 5:00-7:00pm - new families only (ask questions, learn about volunteering using DIBS, working in the concession stands, team game duties, equipment fitting, etc)
  • 6:30-8:00 for all others

Mite (Termite / Supermite) Hockey Registration Instructions (ages 4-8 on June 30th, 2017)


Four Seasons Mite Registration Fees

  • Termites $0 (FREE!!) (Ages 4-6 years on June 30, 2017)*
    • 20 ice hours (in-house)
    • Practice is typically Saturdays 9:30-10:30am (Nov-Feb)
    • Any changes will be posted on DCYH calendar.
    • Jersey to keep
  • Supermites $130 (Ages 7-8 on June 30, 2017)*
    • 30 ice hours (in-house)
    • Practice is typically Wednesday evenings, Satudays 7:50-9:20am and various times on Sunday (Nov - Feb)​
    • Any changes will be posted on DCYH calendar
    • Jersey and socks to keep

*There are no refunds.  The DCYH board reserves the right to apply the payment to next season if a request is made to the DCYH president by December 1st.  


Supermites wanting to move up early to Squirts

If your player is a Supermite with a minimum of two years of playing experience who would like to try out for the Squirts or Girls 10U, we will be having a Supermite tryout.  The date and time will be announced later.  If your child makes the Squirt C or 10U team, he/she will automatically be placed the Squirt C or 10U team.  If your child would like to then try out for the Squirt A or B team, they are welcome to come to the Squirt tryout.  Please register initially as a Supermite.  During registration you will be asked if you would like to do a move up request.  Select "yes" and select "Squirts" or "Girls 10U".  

You will be required to register with USA Hockey and pay the non-refundable $50 USA Hockey fee.  All players, Squirt/10U and above must be registered with USA Hockey. 

For any child wanting to move up a level above their age appropriate level a $25 move up try out fee is required.  Try out fees are not required for kids staying in their age appropriate level (i.e. Squirt wanting to try out for Squirt A's).  If you already did the move up last season (i.e. your supermite moved up to Squirts 2 years early last year), your child is grandfathered in and does not need to attend the supermite try out or pay the fee.  Please register for Squirts and attend the Squirt try outs to try out for the A, B or C team.

Payments can be made after the evaluation once you know which team your child will be playing on.   The $25 move up fee will need to be paid at tryouts.


Youth (10U/Squirt and above) Hockey Registration Instructions

USA HOCKEY REGISTRATION:

Each Player (Squirt/10U and older) must register first with USA Hockey and then return to this Registration Session to signup for the 2017-2018 Season.

  • Step 1: CLICK HERE to go to USA Hockey Registration and register your player(s). Be sure to SAVE their confirmation code(s). Remember to register EACH of your players with USA Hockey and pay the $50 USA Hockey fee. Each player must have their own unique USA Hockey confirmation number.
  • Step 2: Return to this registration and proceed. Later in this registration session you will need to enter the USA Hockey Confirmation Code that you received in Step 1.  Keep for your records.

COPY OF BIRTH CERTIFICATE
A copy of your child's birth certificate is a requirement to play in the Squirt/Girls 10U Level through Bantam Level.  A "Legal" Birth Certificate is the one on file at the courthouse.  A decorative hospital certificate is not acceptable.  The copy of the birth certificate will be shredded after the player's birth date is verified by USA Hockey.  Please do no send the original.  If your child's birth certificate was turned in previously, it will not be needed again.

  • Girls 12U $545 (girls only ages 11 or 12 on June 30, 2017)*
    • 35 ice hours

  • PeeWee $545 (co-ed ages 11 or 12 on June 30, 2017)*
    • 35 ice hours

  • Bantam $605 (co-ed ages 13 or 14 on June 30, 2017)*

Goalies - 12U, Pewee and Bantam goalies will receive a $250 discount on their registration fee.  Goalies are expected to provide their own goalie gear.  Discount only applies to full-time goalies (not back-ups)

* Players will be provided with a home and away jersey that needs to be returned at the end of the season.  Players that lose a jersey will need to replace it at a cost of $60.  Players need to return the jersey in good shape, normal wear is expected.  Each player will receive game socks to keep. 

OMC Sports Med & Fitness Facility is located at 5155 55th Street NW in Rochester.  Players will receive instruction from OMC's hockey specialists and work on skating on OMC's hockey treadmill plus shooting and stick handling on their synthetic ice.  

There are no refunds.   The DCYH board reserves the right to apply the refund to the following season if they so choose if a request has been made to the DCYH president via email by December 1st.


Try out and Move Up Process for Squirt/10U and Above

There is no move up request form this year.  If you would like to move up above your age appropriate level you must meet one of the three conditions below**:  

1)  Birthday  - if your player's birthday is July 1st to September 1st, your player may be allowed to move up a level to stay with other players in his/her grade.  No $25 move up fee is required.  Complete the player's registration for your child's age appropriate level.  When it asks you if you would like to move up, select "yes" and select the move up level. 

2)  Try out * - In order for the player to be allowed to move up a level, the player must try out and make one of the top 10 positions of the highest level or the top goalie position, if available, for the highest level.  (For example, if you have a Squirt age player who would like to play on the Peewees, the Squirt player must tryout and make the one of the top 10 spots on the Peewee A team or top goalie position for the Peewee A team.)  

3)  Grandfathered in - If your child moved up more than one year early last season (ie your player was 7 and played Squirts (age 9-10) in the 2016-2017 season and is now 8 and wants to play Squirts for the 2017-2018 season), no try out fee is required.  When registering, initially select your age appropriate level.  When it asks if you would like to move up and pay the $25 fee, select "yes" and chose the level you are moving up to.  

*All players must have a minimum of two years of playing experience.  You must pay a $25 move up try out fee before try outs.  This is for move ups only. (i.e. Squirt age wanting to move up to Peewee.  If your player is a Squirt for example, wanting to try out for Squirt A or B, no fee is required.)  Register for your age appropriate level.  If the player ends up moving up a level, the registrar will adjust your registration.  Please pay $25 tryout fee at first tryout session. An email will go out with the October try out dates and times.*

**All approvals for early move ups are only allowed if availability on a team and are subject to approval by the Director of Hockey and the DCYH Oversight Group.**

 


Youth Tournaments

  • Each team Squirt/Girls 10U through Bantam will attend at least two tournaments.  DCYH will pay $2,000 per team for gate fees and tourney fees (use it or lose it).  Any amount over the $2,000 will need to be paid by the team and reimbursed to DCYH.  
  • If a team Squirt/Girls 10U through Bantam level qualifies for a district, regional or state tournament the registration fee will be covered by DCYH.  Gate fees need to be paid by the team.
  • Parents will be responsible for travel and lodging expenses.

2017 - 2018 Volunteer Deposit / Hours Requirements

  • Termite
    • $100 volunteer deposit paid with registration
    • 6 volunteer hours
  • Supermite
    •  $150 volunteer deposit paid with registration
    • 9 volunteer hours
  • Youth (Squirt/10U and above)
    • $275 volunteer deposit paid with registration
    • 14 volunteer hours

Families with multiple players will be responsible for the volunteer deposit amount designated for each child, with a family max deposit of $400.  Families with multiple skaters will be required to complete the designated volunteer hours for the child registered at the highest level and 6 extra hours for each additional child. 

A check for Volunteer Deposit post dated 3/1/2018 must be turned in to DCYH prior to your skater's first practice for the 2017-18 season for them to be allowed to participate.

Checks will not be cashed unless the hours are not fulfilled by the end of the season. Volunteer deposits will only be returned if ALL hours are completed and you pick them up at the annual board meeting in April.  All others will be destroyed.  All volunteer hours will be posted on our website under DIBS throughout the season for you to claim to fulfill your hours.


2017-2018 Fundraising Requirements

Fundraising is a vital function of the DCYH program.  The proceeds generated  are crucial in keeping costs affordable.  All skaters are required to participate in a youth hockey fundraiser or pay a buy-out fee determined by DCYH.

Fundraiser Requirements/Options

Each skater must complete one of the following options by December 1st, 2017 and there is no pro-rating, which means you can't sell half of the pizza requirement and then pay half of the cash buyout requirement to meet a full requirement for one skater.*

  1. Pizza Sales (More Information): Sell $400.00 in merchandise starting November 1st (pizza's, cookies, etc.)
  2. Cash Buyout: $200.00 (checks will be deposited on December 1st)
  3. Kwik Trip Ultimate Car Wash Cards (More Information): Sell $468.00 (13 total cards (5 washes per card))
  4. Kwik Trip Gift Cards (More Information)Sell/Buy $4000 in cards 

*For families with multiple skaters, each skater over 2 will receive a 50% discount on the required amounts above.  See example below:

Example: Smith family has three children participating in DCYH.

  • Child #1: Bantam, Family sells $400 in pizza.
  • Child #2: 12U, Family sells $468 in Kwik Trip Ultimate Car Wash Cards
  • Child #3: Termite, Family does buyout for $100 (50% of buyout option on child #3)

Registration fee payment (or post-dated checks if paying in installments), birth certificate (if not previously submitted), consent to treat form and volunteer deposit checks are due by the first practice/first day of try-outs or the player will not be allowed on the ice.  Fundraiser requirements must be met by December 1st or your child will be removed from the official USA Hockey roster and is no longer allowed on the ice.

Also, please read the DCYH Parent and Player Code of Conducts before the season begins and discuss them with your spouse, family and players.  These are found on the DCYH website in the "Parents" section.  All players and parents are required to comply with our Code of Conducts.  When registering your child for hockey with DCYH both parents and players agree to comply with our Code of Conducts or be subject to a penalty to be determined by the DCYH discipline committee or board.

Equipment Rental Form

$40 Annual Equipment Rental Fee for Skaters (or $6 per item)

Goalies Free (Supermite and Squirt Only)

Goalies Pewee, 12U and Bantam are to provide their own gear.  A $250 registration fee discount will apply to accomodate this.  Discount only eligible to Pewee, 12U and Bantams who are full-time goalies.

First time renters or those currently renting and in need of different equipment, should visit the Dodge County Ice Arena on one of the equipment rental/fitting days listed below.

Players keeping rented equipment for the upcoming season need to print and complete the Equipment Form attached and submit payment payable to DCYH.

All players will be provided with jerseys.
